Did you purchase it from T-Mobile, direct from OnePlus, Amazon. BestBuy, or somewhere else? If it's an international model. That might be why.
It was purchased from OnePlus.

That is more than likely why then. The device IMEI is not in the T-Mobile system to accept for a trade-in. Try reaching out to T-Mobile via DM on Facebook or Twitter to see if they can manually process the trade for you.

I'm having the same problem, getting IMEI not recognized. KeithB, did you resolve yours?
While my phone (Moto G) wasn't bought through T-mobile, the explanation that it's an IMEI unknown to T-mobile does not hold water, because this phone was literally on my tmobile plan for nearly 5 years! It's an old phone, but I'm trying to buy an upgrade using the P772 promotion which gives credit for ANY trade in on certain upgrades, so I pulled this out of drawer where its been sitting since maybe 2017 ... the trade in entry acts like it will accept it (though will offer $0 for it lol) but it has the option to trade it in anyway, which I want to do for P772 terms, but then it asks for the IMEI, and rejects this code for a phone that spent years on its network. I'm 100% certain I typed it in correctly.

Used on the network doesn't matter. That IMEI isn't stored. The identifier for your current device is just that. The IMEI would be stored in T-Mobile's system in a different way if you purchased the device from T-Mobile.
