I'd cancel your existing service with T-Mobile, then reestablish it at the lower rate. If they question why you're doing it, tell them that’s the only way you qualify for the lower rate. My bet is a supervisor will lower your monthly bill to $30 rather than go through the bother of returning the old modem and then sending out a new one to reestablish service.
