I'm being double charged

I'm being charged for two lines and have been for months when I only have one. I have called multiple times to request this be fixed and it never has been. Now my account is locked because I lost my phone and I’m hoping to get this resolved prior to reinstating my account. Can someone please, please actually help me with this?
