2G shutdown free phone replacement

Is T-Mobile still offering the 2G free phone replacement because of the network shutdown? What do I need to do to qualify for this program?

You should of received an email from Tmobile if you were affected, you can always sign into your MyTmobile account and check to see what phones their offering for free with bill credits.
