network pass

I have a network pass 3 months free on my iPhone so I erased my iPhone last night & the cellular plan got erased as well which I can’t get it back on my phone please can you help

    It’s active on my T-Mobile application but not on my phone settings it’s asking me to add a new cellular plan & I don’t have the information required
