switch to T-Mobile bonuses

I switched from AT&T to T-Mobile and was promised $200 per phone bonus in the form of T-Mobile credit. It was supposed to be sent to me by email in 6 to 8 weeks. I've tried to contact my sales person and gotten no return calls or emails. It's been 6 months!

Anyone else have this problem? Any suggestions on how I can get the bonus?
