Where to get QR code to restore deleted T-Mobile entry in Authenticator app

I am trying to switch T-Mobile authentication to 1Password from Google authenticator. I had to delete from Google authenticator but I cannot find where to get the QR code or setup key to have authentication in 1Password. I am stumped.
