Insurance Claim

I'm putting in an insurance claim with my insurance for a BYOD device. How do I show that my prepaid account did not provide the phone? They say it usually has something like a BYOD indicator somewhere in my account. I have looked in all the screens and can't find it. Anyone know where to find this?
