Only the [Primary Account Holder](https://www.t-mobile.com/support/account/set-up-line-permissions) or an authorized user can file a claim. Claims must be reported within 90 days of the incident unless state law requires additional time.

If you have AppleCare, please contact [Official Apple Support](https://support.apple.com/) for accidental damage or file a claim as outlined on this page for loss and theft.
### You will need the following:
- Your mobile number
- The make and model of your device
- The date the incident occurred, including the last time you [used the device](https://www.t-mobile.com/support/account/check-your-usage).
- Additional information may be required, such as:
- Proof of ownership and/or a government-issued ID
- If stolen, the police report number. Learn how to [report your phone as lost or stolen](https://www.t-mobile.com/support/account/lost-or-stolen-device-help) to prevent unauthorized usage.
### P360 app claim
1. Open the [P360 app by Assurant](https://install.myp360.com/).
2. Select __File a Claim__.
3. Make sure you have the information required, then choose __Get Started__.
4. Follow the step-by-step instructions presented in the app.

### Online claim at mytmoclaim.com
1. Visit [mytmoclaim.com](https://mytmoclaim.com/) using a web browser.
2. Select __File or Track a Claim__.
3. Follow the step-by-step instructions presented in the app. You will need to log with your [T-Mobile ID](https://www.t-mobile.com/support/account/set-up-and-manage-your-t-mobile-id).

### Online claim in your T-Mobile Account
1. Log in to your [T-Mobile account](https://account.t-mobile.com/) using a web browser.
2. Select __Account__ and choose the line you're filing a claim on.
3. Select __File a claim__ or __File damage claim__ or __Report lost or stolen__. A details window appears providing the next steps.
4. Once you confirm the selection, you'll be redirected to the claims website at [](https://www.t-mobile.com)[mytmoclaim.com](https://mytmoclaim.com/).
5. Ensure the mobile number is the correct one for the damaged, lost, or stolen device. If it isn't, return to the __Accounts__ screen and choose the correct mobile number.
6. Follow the step-by-step directions on the Assurant website to file your claim.
